Obverse: Die gouge touches the bottom right of the N in IN. Curved die gouge above the Y in LIBERTY. Die gouge to the right of the 3 in the date. Die gouge to the right of the mintmark P.
Reverse: Reverse is SDDR-016, the ceiling of the doorway is slightly doubled to the south. Die gouge between the T and I in MONTICELLO.
